Painful patterns can change
Some people feel that their life keeps repeating the same painful story. Relationships begin with intensity and end in hurt. Emotions swing so fast that others cannot keep up. Anger comes before thought, or a deep emptiness follows every success. Many are told they are dramatic, selfish or impossible. Family members are exhausted and confused and often blame themselves or each other.
A personality disorder is not a judgment of character. It describes long standing patterns of thinking, feeling and relating that cause lasting distress or damage to relationships and daily life. These patterns usually begin in the teenage years or early adulthood and are shaped by temperament, early experiences and stress. The good news is that they are not fixed. With the right therapy, steady support and, where needed, medicine for specific symptoms, many people learn to manage emotions, build stable relationships and live more fully.
This page explains the main types of personality disorder, how a clinician makes the diagnosis and what treatment involves. If you want to speak to someone now, Talk to our admissions team on WhatsApp. Your message is confidential.
What is a personality disorder
Everyone has a personality, which is the way we usually think, feel and behave. A personality disorder is diagnosed only when the pattern is:
- Long lasting. It has been present since adolescence or early adulthood.
- Rigid. It shows up in many settings, such as home, work and friendships.
- Distressing or damaging. It causes real problems for the person or for the people around them.
- Not better explained by something else. For example, it is not just the effect of drugs, a medical illness or another mental health condition.
A personality disorder can only be diagnosed by a qualified clinician after a careful assessment, usually in adults. Online quizzes and social media descriptions cannot diagnose it, and the words “narcissist” or “borderline” are often used loosely in daily life.
Types of personality disorder
Personality disorders are grouped into three clusters.
Cluster A
odd or eccentric
Paranoid, schizoid and schizotypal personality disorder. People may be very suspicious, detached or have unusual beliefs.
Cluster B
dramatic, emotional or unpredictable
Borderline, narcissistic, histrionic and antisocial personality disorder.
Cluster C
anxious or fearful
Avoidant, dependent and obsessive compulsive personality disorder.
The two that people search for most often are borderline and narcissistic personality disorder, described below.
Borderline personality disorder
Borderline personality disorder, often shortened to BPD, is marked by intense and unstable emotions, relationships and sense of self. The National Institute of Mental Health describes it as a mental illness that severely impacts a person’s ability to regulate their emotions. Common features include:
- Intense fear of abandonment, real or imagined
- Relationships that swing between idealizing and rejecting the other person
- An unstable sense of who you are
- Strong mood swings that last hours to days
- Impulsive actions such as reckless spending, risky behavior or substance use
- Chronic feelings of emptiness
- Intense anger that is hard to control
- Self harm or suicidal thoughts and behavior
- Brief episodes of feeling suspicious or detached from reality during stress
BPD is often missed or confused with depression, bipolar disorder or anxiety, and in women it is frequently mistaken for being “too emotional”. Early and accurate assessment makes a real difference. With specialist therapy, many people improve substantially over time.
Narcissistic personality disorder
Narcissistic personality disorder involves a long term pattern of needing admiration, a feeling of being special or superior, and difficulty with empathy. Common features include:
- A strong sense of self importance
- A need for constant admiration or attention
- Sensitivity to criticism, with anger, shame or withdrawal
- Taking advantage of others, often without noticing
- Difficulty recognizing the feelings and needs of other people
- Envy, or a belief that others are envious
- Underneath, often fragile self esteem
Not everyone with narcissistic traits has the disorder, and people with it are often distressed by relationship failures, work problems, depression or substance use rather than by the personality pattern itself. This is usually the point when they come for help.
Other personality disorders
- Avoidant and dependent patterns. Deep fear of rejection, social withdrawal and difficulty making decisions alone.
- Antisocial patterns. Disregard for the rights of others, impulsivity and rule breaking.
- Obsessive compulsive personality disorder. Rigid perfectionism and control, which is different from OCD. See our OCD Treatment page.
- Paranoid, schizoid and schizotypal patterns. Mistrust, detachment or unusual thinking.
A psychiatrist and psychologist will assess which, if any, fit your situation.
Personality disorder, or something else
Many other conditions look similar. Bipolar disorder, depression, anxiety, trauma related conditions, ADHD, substance use and psychosis can all cause mood swings, anger, impulsivity or unstable relationships. They also often occur together with a personality disorder. A proper assessment looks at all of these. See our Bipolar Disorder Treatment, Depression Treatment and Anxiety Disorder Treatment pages.
Causes and risk factors
There is no single cause. Personality disorders are linked to a mix of:
- Temperament, meaning how sensitive or reactive a person is from childhood
- Family history of mental illness
- Childhood neglect, abuse, loss or an unstable home
- Growing up where emotions were punished or ignored
- Trauma and long term stress
- Alcohol, tablets or drug use, which can worsen symptoms
These are risk factors, not blame. Many people with a difficult childhood do not develop a personality disorder, and many with the disorder did their best in hard circumstances.
Is it time to get help?
Consider an assessment if:
- Relationships keep ending painfully in similar ways
- Emotions or anger feel out of control
- You act on impulse and regret it later
- You feel empty, or unsure who you are
- You have harmed yourself or thought about it
- Family or friends say they no longer know how to help
- You are using alcohol, tablets or drugs to cope
- Depression, anxiety or addiction has not improved with earlier treatment
If you or someone near you is thinking about suicide or self harm, treat it as an emergency. Go to the nearest emergency department now and do not leave the person alone.
How clinicians assess a personality disorder
Diagnosis is not made in one short visit. It involves:
- A detailed conversation about your life history, relationships, work and emotions
- A review of how long the patterns have been present
- Questions about mood, anxiety, trauma, substance use and thoughts of self harm
- Structured clinical interviews or questionnaires where suitable
- With your agreement, information from a family member
- A medical check, so that physical causes are not missed
We take time over this because a rushed label can harm. The aim is an accurate, respectful understanding that leads to a plan.
how we work ?
Our personality disorders Treatment Program in Islamabad
FCRC’s anxiety treatment program follows six steps clear clinical framework, personalised to every patient.
Private assessment
A psychiatrist and a clinical psychologist assess you over more than one session where needed. You are listened to without judgment.
Diagnosis and explanation
You are told what the assessment shows, in plain language. Many people feel relief at having words for what they have lived with.
Safety and stability
If there is a risk of self harm, or a crisis, the first step is safety. We agree a plan with you and, if you wish, your family. Residential care is available when needed.
Specialist therapy
Therapy is the main treatment for personality disorders. Our psychologists use dialectical behavior therapy (DBT), a skills based therapy designed for intense emotions, together with long term talking therapy and CBT, to help you understand emotions, manage intense feelings, reduce impulsive actions, handle conflict and build steadier relationships. See our Counseling and Therapy and CBT pages.
Psychiatric care for specific symptoms
No medicine treats a personality disorder itself. But a psychiatrist can treat conditions that occur with it, such as depression, anxiety, severe mood swings or sleep problems, and can review any medicine you already take.
Family support and follow up
Our Family Support Program helps relatives understand the condition, set healthy limits and communicate more calmly. Follow up visits and our Aftercare Planning keep progress going.

A Place Built for Healing
Dialectical behavior therapy (DBT)
DBT is a structured therapy that was developed for people with borderline personality disorder and is now widely used for intense emotions, impulsive behavior and self harm. The word dialectical points to its main idea: accepting yourself as you are while also working to change. It teaches four groups of skills:
- Mindfulness. Staying in the present and noticing thoughts and feelings without being swept away by them.
- Distress tolerance. Getting through a crisis without making it worse, for example without self harm, substance use or an outburst.
- Emotion regulation. Understanding emotions, reducing how vulnerable you are to them and changing painful ones.
- Interpersonal effectiveness. Asking for what you need, saying no and keeping relationships steady.
Skills are practiced in sessions and between sessions, so they become habits. DBT is often combined with individual therapy and psychiatric care, and family members can learn the same skills through our Family Support Program. It also helps people with other long standing patterns, such as anger, impulsivity and substance use.
Therapy for personality disorders
Personality disorders change slowly, and therapy works best when it is regular and continues for months rather than weeks. Helpful therapy usually includes:
- Learning to notice and name emotions before acting on them
- DBT skills for calming strong feelings and tolerating distress
- Understanding how early experiences shaped patterns
- Practicing new ways of dealing with conflict and closeness
- Building a stable routine, sleep and daily structure
- Reducing alcohol, drug or tablet use
Progress is rarely a straight line. Setbacks are part of the process, and a steady therapeutic relationship is one of the most important parts of treatment.
How to choose treatment for a personality disorder in Islamabad?
- Choose a service with a qualified psychiatrist and clinical psychologist who assess you properly before giving a diagnosis.
- Ask what therapy is offered and how long it lasts. Quick fixes do not work for long standing patterns.
- Be careful of anyone who promises a rapid cure, or who blames you or your family.
- Check how safety and self harm are managed.
- Ask how family members will be supported.
- Check that addiction and other conditions are screened.
Why choose FCRC for personality disorder treatment in Islamabad?
Because trust, experience and results matter most.

Outpatient or residential care
Some people do well with outpatient visits and regular therapy. Residential care is considered when there is a risk of self harm, when symptoms are severe, when addiction is also present, or when a calm place is needed to start therapy. See our Outpatient Rehab Program and Inpatient Rehab Program pages
Medicine and personality disorders
- There is no medicine that cures a personality disorder.
- Medicine may help with specific symptoms such as depression, anxiety, mood swings or sleep, and is prescribed only after assessment.
- Do not rely on sleeping tablets, sedatives or painkillers to cope with emotional pain. These can cause dependence. See our Xanax and Tramadol Addiction Treatment and Pregabalin Addiction Treatment pages.
- Do not start, stop or change medicine without your psychiatrist.
Personality disorder and addiction
Alcohol, tablets and drugs are common in people with personality disorders, often as a way to quiet pain or impulses, and they make symptoms worse. Treating only one leaves the other in place. Our Dual Diagnosis Treatment page explains how we treat both together.
Supporting a family member
Living with someone who has a personality disorder can be exhausting. You can help by:
- Staying calm, and talking about difficult topics when emotions are lower
- Setting clear, kind and consistent limits
- Taking threats of self harm seriously and getting help right away
- Avoiding blame, name calling and arguments in the heat of the moment
- Encouraging treatment without forcing it
- Looking after your own wellbeing, and getting support for yourself
Women and young people
Personality disorders, particularly borderline patterns, are often missed in women and diagnosed late. Young people with intense emotions or self harm need careful assessment and not a quick label. Tell us at first contact so that the right clinician is involved.
What affects the cost of Personality disorder treatment?
We do not publish a fixed price here, because it depends on your needs. The main factors are:
- Whether you need outpatient visits or residential care
- How many assessment, therapy and family sessions are recommended
- Length of treatment, which is usually longer for personality disorders
- Tests and medicine required
- Any other condition that needs treatment at the same time
Message us on WhatsApp and the admissions team will explain the cost clearly before you decide.
frequently asked question
What is the best treatment for a personality disorder?
Long term therapy is the main treatment, and DBT is one of the best studied therapies for borderline personality disorder. Medicine may help with specific symptoms that occur alongside it. A psychiatrist and psychologist plan this after a careful assessment.
What is borderline personality disorder?
BPD is a condition marked by intense and unstable emotions, relationships and sense of self, with fear of abandonment, impulsive behavior and sometimes self harm.
What are the symptoms of BPD in women?
Symptoms are the same in men and women, but women are more often diagnosed. They may include intense mood swings, fear of abandonment, anger, emptiness and self harm. Many are first told they have depression or anxiety.
What is narcissistic personality disorder?
It is a long term pattern of needing admiration, feeling superior and struggling with empathy. People with it often seek help for another problem such as depression or relationship breakdown.
Can a personality disorder be cured?
Personality disorders are long standing, but they are not fixed. Many people improve a great deal with therapy over time and learn to manage their emotions and relationships.
How long does personality disorder treatment take?
Longer than for many other conditions. Therapy usually continues for months and sometimes longer. Your clinicians will explain what to expect.
What is DBT, and can it help with BPD?
DBT, or dialectical behavior therapy, is a skills based therapy that teaches mindfulness, distress tolerance, emotion regulation and interpersonal skills. It was developed for borderline personality disorder and is one of the best studied treatments for it.
Is there medicine for personality disorder?
No medicine treats the disorder itself, but medicine can treat depression, anxiety, mood swings or sleep problems that occur with it.
Can I diagnose myself with an online test?
No. Online tests and social media can raise questions, but a diagnosis needs a careful assessment by a qualified clinician.
Is a personality disorder the same as bipolar disorder?
No. Bipolar disorder involves distinct episodes of high and low mood and responds to different treatment. They can look alike, so a clinician needs to assess carefully. See our Bipolar Disorder Treatment page.
Can someone with a personality disorder be forced into treatment?
Treatment works best when the person agrees. Families can contact us for guidance on how to approach the conversation.
Do you treat personality disorder together with addiction?
Yes. We treat them under one plan. See our Dual Diagnosis Treatment page.
What should I do if someone is threatening self harm?
Treat it seriously. Stay with the person, remove any means of harm if it is safe to do so and get them to the nearest emergency department.
