Mental Health in Pakistan: The Reality

According to the World Health Organization, one in eight people globally lives with a mental health disorder. In Pakistan, estimates suggest that more than 50 million people are affected by some form of mental illness, yet the treatment gap, the proportion of those who need care but do not receive it; remains among the highest in the world.

The consequences of untreated mental illness are profound. Untreated depression is a leading driver of suicide, disability, and substance abuse. Untreated anxiety destroys careers, relationships, and quality of life. Untreated psychosis causes irreversible harm if not managed promptly and expertly. Depression

Depression is one of the most prevalent and most misunderstood mental health conditions in Pakistan. According to the Mayo Clinic, major depressive disorder involves persistent low mood, loss of interest and pleasure, fatigue, sleep disturbance, and in severe cases, suicidal ideation. It is a medical condition with established, effective treatments — not a character weakness or a spiritual failing.

At FCRC, depression is treated with a combination of accurate psychiatric diagnosis, carefully managed medication where clinically indicated, and evidence based psychological therapy including CBT and other validated approaches. Our treatment plans are fully personalised and reviewed regularly throughout the treatment period.

Anxiety Disorders

Anxiety disorders are the most common category of mental health conditions globally, and Pakistan is no exception. Generalised anxiety disorder, panic disorder, social anxiety disorder, and health anxiety all produce significant suffering and functional impairment when left untreated.

FCRC’s consultant psychiatrists have extensive experience managing psychotic illness, including accurate differential diagnosis between primary psychotic disorders and substance induced psychosis, antipsychotic medication management, and the psychological and family support that sustained recovery from psychosis requires.

Post Traumatic Stress Disorder (PTSD)

PTSD develops in response to traumatic experiences and produces a characteristic cluster of symptoms including intrusive memories, nightmares, hypervigilance, emotional numbing, and avoidance of trauma reminders. According to the American Psychological Association, evidence based treatments for PTSD including trauma focused CBT and EMDR are highly effective when delivered by trained clinicians.

Dual Diagnosis — Mental Health and Addiction

A significant proportion of patients at FCRC present with both a mental health condition and a substance use disorder. Every patient who comes to FCRC for mental health care begins with a thorough psychiatric assessment conducted by our consultant psychiatrists. This covers the patient’s full mental health history, current symptoms, physical health, family history, social circumstances, and personal goals.

Accurate diagnosis is the foundation of effective treatment. At FCRC, we take the time that a thorough psychiatric assessment requires, and we never rush a clinical judgement that has lasting consequences for the patient.

Personalised Treatment Planning

Following assessment, a fully personalised treatment plan is developed by the clinical team. This plan specifies the combination of psychiatric management, psychological therapy, and support services most appropriate for the individual patient’s presentation, and is explained clearly to both the patient and, where appropriate, their family.

Treatment plans at FCRC are living documents, reviewed and adjusted regularly in response to the patient’s progress and evolving needs.

Psychiatric Medication Management

Where medication is clinically indicated, FCRC’s consultant psychiatrists manage it with the rigour and precision that psychiatric pharmacology requires. Medication is never the only component of treatment at FCRC, but where it is part of the treatment plan, it is managed expertly, with regular review, clear communication with the patient, and careful monitoring for side effects and therapeutic response.

Evidence Based Psychological Therapy

Alongside psychiatric management, FCRC’s clinical psychologists and therapists deliver evidence based psychological therapies tailored to the patient’s specific condition and needs.

Cognitive Behavioural Therapy (CBT) is the most extensively evidenced psychological treatment for depression, anxiety, OCD, and many other conditions. At FCRC, CBT is delivered by trained clinical psychologists and adapted to the individual patient’s presentation.

Dialectical Behaviour Therapy (DBT) is particularly effective for patients with emotional dysregulation, self harm behaviours, and borderline personality disorder. FCRC’s DBT programme combines individual therapy with skills training in mindfulness, distress tolerance, emotional regulation, and interpersonal effectiveness.

Trauma Informed Therapy provides a safe, carefully paced therapeutic space for patients whose mental health conditions are rooted in traumatic experiences. At FCRC, trauma work is conducted with clinical sensitivity and at the patient’s own pace.

Motivational Interviewing supports patients in building their own internal motivation for change, particularly valuable for patients whose mental health condition is connected to substance use or lifestyle patterns that sustain their difficulties.

Family Involvement and Psychoeducation

Mental health conditions affect families, not just individuals. At FCRC, family members are involved in the treatment process through structured psychoeducation sessions that help them understand the nature of their loved one’s condition, family therapy where clinically indicated, and practical guidance on how to support recovery without inadvertently sustaining unhealthy patterns.

Aftercare and Follow Up

Mental health recovery is a long term process. Before every patient completes their programme at FCRC, a comprehensive aftercare plan is developed, covering ongoing medication management, follow up therapy appointments, and the support structures needed to sustain recovery in daily life.

FCRC’s clinical team remains available to all former patients for follow up consultation and support after discharge.

[Outpatient psychiatric consultation

is available for patients who require professional psychiatric assessment, diagnosis, and ongoing management while continuing to live at home. Outpatient appointments are available on a scheduled basis at FCRC’s Bani Gala facility, with flexibility in appointment frequency based on clinical need.

Inpatient and Outpatient Mental Health Services

FCRC offers mental health and psychiatric treatment through both residential inpatient and structured outpatient formats.

Inpatient psychiatric care

at FCRC is available for patients whose condition requires the structure, safety, and intensive clinical support of a residential setting. This includes patients with severe depression, acute psychosis, complex dual diagnosis presentations, and those for whom the home environment is not safe or supportive of recovery. Patients live within FCRC’s Bani Gala facility under 24/7 clinical supervision, with a structured daily programme of psychiatric management, therapy, and wellness activities.
