Stopping alcohol after prolonged heavy drinking is not simply a matter of deciding to stop. For a significant proportion of people with alcohol addiction, the withdrawal process is a medical event — one that can range from deeply uncomfortable to genuinely life-threatening depending on the severity of the dependency and whether it is managed with appropriate clinical support.

This is one of the most important and least understood facts about alcohol addiction. Families often assume that stopping drinking is the hard part and that once the person has stopped, the worst is over. In reality, for someone with severe alcohol dependency, the act of stopping without medical supervision can be more dangerous than continuing to drink.

Understanding what alcohol withdrawal is, why it happens, what the symptoms look like, and why medical management is essential is critical for anyone dealing with alcohol addiction in themselves or in a family member.

Why alcohol withdrawal happens

To understand withdrawal, it is necessary to understand what sustained heavy alcohol use does to the brain.

Alcohol is a central nervous system depressant. It slows down brain activity by enhancing the effect of GABA, the brain’s primary inhibitory neurotransmitter, and by suppressing glutamate, the brain’s primary excitatory neurotransmitter. The net effect is the sedation, slowed thinking, and reduced inhibition that characterise intoxication.

With sustained heavy drinking, the brain adapts to the constant presence of alcohol. Because alcohol is constantly suppressing neural activity, the brain compensates by upregulating its excitatory systems and downregulating its inhibitory systems, trying to maintain normal function in the presence of a depressant. It produces more glutamate receptors and reduces GABA activity to counterbalance the alcohol’s effect.

When alcohol is then removed, the brain is left in a state of profound excitatory imbalance. The suppressive effect of alcohol is gone, but the compensatory upregulation of excitatory activity remains. The brain is now in a hyperexcitable state, which is the neurological basis of alcohol withdrawal. This hyperexcitability is what produces the symptoms of withdrawal and in severe cases, it is what makes those symptoms life-threatening.

This is why alcohol withdrawal is fundamentally different from the withdrawal of substances like cannabis or opioids. Opioid withdrawal is agonising but not typically fatal in otherwise healthy adults. Alcohol withdrawal, in severe cases, can kill. The brain’s hyperexcitable state can produce seizures and a condition called delirium tremens that carries a significant mortality risk if untreated.

The timeline of alcohol withdrawal symptoms

Alcohol withdrawal follows a relatively predictable timeline, though the severity varies enormously depending on how much the person has been drinking, for how long, their age and general health, and whether they have experienced previous withdrawal episodes.

Six to twelve hours after the last drink:

The earliest symptoms of withdrawal typically begin within six to twelve hours of the last drink. At this stage they are uncomfortable but not dangerous for most people. They include:

  • Anxiety and agitation
  • Tremor, particularly in the hands
  • Nausea and vomiting
  • Headache
  • Sweating
  • Elevated heart rate and blood pressure
  • Difficulty sleeping
  • Sensitivity to light and sound

For people with mild to moderate dependency, withdrawal may not progress significantly beyond this stage. For those with severe dependency, these early symptoms are the beginning of a potentially serious medical progression.

Twenty-four to forty-eight hours after the last drink:

In the second day of withdrawal, the risk of seizures is highest. Alcohol withdrawal seizures are grand mal seizures, involving loss of consciousness and convulsions, and they can occur without warning even in people who have never had a seizure before. They are a direct consequence of the brain’s hyperexcitable state and are a medical emergency requiring immediate clinical management.

Not everyone in withdrawal will have a seizure. The risk is substantially higher in people who have undergone multiple previous withdrawals, a phenomenon known as the kindling effect, in which each successive withdrawal episode produces more severe symptoms than the previous one.

Forty-eight to seventy-two hours and beyond:

The most severe complication of alcohol withdrawal is delirium tremens, commonly known as DTs. It typically develops between 48 and 72 hours after the last drink, though it can occur later, and represents a life-threatening medical emergency.

Delirium tremens is characterised by:

  • Severe confusion and disorientation
  • Vivid and terrifying hallucinations, typically visual — seeing insects, animals, or people that are not present
  • Profound agitation and autonomic instability
  • Dangerously elevated heart rate, blood pressure, and body temperature
  • Seizures

Without treatment, delirium tremens carries a mortality rate of up to 37%. With appropriate medical treatment, that figure drops dramatically to around 1 to 5%. According to the World Health Organization, medically supervised alcohol detoxification is essential precisely because of this risk. The difference between treated and untreated severe withdrawal is not a matter of comfort. It is a matter of survival.

Who is at highest risk of severe withdrawal

Not every person who stops drinking will experience severe withdrawal. Understanding the risk factors helps families and individuals assess how urgent medical support is.

Duration and quantity of drinking. The longer and more heavily a person has been drinking, the more profound the brain’s compensatory adaptation and the more severe the withdrawal is likely to be.

Previous withdrawal episodes. The kindling effect means that each previous withdrawal primes the brain to respond more severely to the next one. A person who has undergone multiple withdrawals is at significantly higher risk of seizures and delirium tremens than someone experiencing their first withdrawal.

Age and general health. Older individuals and those with compromised liver function, cardiovascular disease, or other medical conditions are at higher risk of complications.

Concurrent use of other substances. The use of benzodiazepines, other sedatives, or stimulants alongside alcohol complicates the withdrawal picture and increases risk.

Previous seizure history. Any personal history of seizures is a significant risk factor during alcohol withdrawal.

If any of these risk factors are present, medically supervised detoxification is not simply recommended. It is essential.

Alcohol addiction and alcohol abuse: understanding the spectrum

Alcohol withdrawal occurs in the context of alcohol addiction or severe alcohol abuse. Understanding where a person sits on the spectrum of alcohol use disorder is part of understanding what level of support they need.

Alcohol abuse, now more commonly referred to as harmful alcohol use, describes a pattern in which alcohol consumption is causing harm to the person’s physical health, mental health, relationships, or functioning, but in which the full features of dependency are not yet present. The person may be drinking heavily and experiencing consequences, but still retains meaningful control over their use.

Alcohol addiction, clinically known as alcohol use disorder, involves the loss of that control. The person continues to drink despite serious consequences, experiences craving and preoccupation with alcohol, finds it increasingly difficult to function without alcohol, and develops the physical dependency that produces withdrawal symptoms when drinking stops.

The World Health Organization estimates that harmful alcohol use causes approximately 3 million deaths globally each year and is a leading risk factor for disability. While alcohol use in Pakistan is lower than in many other countries due to cultural and religious factors, it is not absent, and the consequences for those who do develop alcohol use disorder are as serious here as anywhere.

The stigma attached to alcohol use in Pakistan adds a specific dimension. People with alcohol use disorder in Pakistan often drink in secrecy, do not disclose use to healthcare providers, and do not seek help until the condition is severe. This delay compounds both the medical risk of withdrawal and the difficulty of treatment.

Why alcohol withdrawal must be medically managed

The severity of alcohol withdrawal means that attempting to stop drinking without medical supervision carries real risk for anyone with significant dependency. This is not a matter of preference or comfort. It is a clinical necessity.

Medical management of alcohol withdrawal serves several functions.

Seizure prevention and management. Benzodiazepine medications, which enhance GABA activity and thereby counteract the brain’s excitatory imbalance, are the primary pharmacological tool for preventing and managing withdrawal seizures. They are titrated against the severity of withdrawal symptoms using validated clinical scales and are reduced gradually as the withdrawal resolves. This is a medical process that requires clinical judgement and monitoring.

Management of delirium tremens. Delirium tremens require intensive medical management including intravenous fluids, electrolyte correction, high-dose benzodiazepines, and close monitoring of vital signs. It cannot be safely managed outside a clinical setting.

Nutritional support. Prolonged heavy alcohol use is associated with severe nutritional deficiencies, particularly thiamine (Vitamin B1). Thiamine deficiency during withdrawal can cause Wernicke’s encephalopathy, a serious neurological condition, and its prevention requires thiamine supplementation as a routine component of medically managed detoxification.

Monitoring and safety. Regular monitoring of vital signs, mental state, and withdrawal severity allows clinical staff to adjust treatment in real time and to respond promptly to any deterioration.

Psychological support. The acute phase of alcohol withdrawal is a period of significant psychological distress. Medical staff who understand both the physical and psychological dimensions of withdrawal provide support that makes the process more bearable and that reduces the risk of premature departure from treatment.

What comes after detoxification

A significant portion of their time is spent drinking, recovering from drinking, or planning when they will next drink. Obligations at work, home, or in relationships are consistently neglected because of drinking. The person continues to drink despite clear harm to their health, relationships, or functioning. They need to drink to feel physically normal and experience distress or physical symptoms when they do not drink.

If these features are present, what you are observing is not a drinking problem that willpower can fix. It is a medical condition that requires professional treatment.

When to seek help

If you or someone you care about is drinking heavily and daily, and particularly if stopping has been attempted and produced physical symptoms or if there is any history of seizures during previous attempts to stop, seeking medical assessment before attempting to stop is essential.
